Metro Energy Investments, LLC provides renewable energy projects which enable investors to participate in membership interest and the tax benefits associated with renewable energy investments. Renewable energy investments allow investors to diversify and reinforce their portfolios with a commodity that is increasing in demand. In addition, investors have the unique opportunity to play a key role in promoting our country's energy independence through utilizing the latest technologies and advancements made in solar, geothermal, and wind. Please fill out the questionnaire below if you're interested in determining if you qualify for renewable energy investments with our company.

A. ALL ACCREDITED INVESTORS—If you believe that you are an "Accredited Investor" within the meaning of Rule 501 of Regulation D, please indicate, by initialing the appropriate paragraph below, the category or categories that apply. (A definition of "Accredited Investor," as defined in Regulation D, is attached for your reference.)
1. As of the date of my purchase, I individually or with my spouse have a net worth in excess of $1,000,000, excluding primary residence; or
2. I had an individual income in excess of $200,000 in each of 2010 and 2011 and reasonably expect an individual income in excess of $200,000 in 2012; or
3. My spouse and I had a joint income of $300,000 in each of 2010 and 2011 and reasonably expect an income in excess of $300,000 in 2012; or
4. The investor is an organization or person who qualifies as an Accredited Investor under Rule 501(a).

B. CALIFORNIA ACCREDITED INVESTORS ONLY—For purposes of categories (1), (2) and (3) of Section 4A above, you will be considered an accredited investor under the rules of the California Commissioner of Corporations under Section 25102(f) of the California Corporate Securities Law only if you also meet one of the tests set forth below. Therefore, please indicate, by initialing the appropriate paragraph or paragraphs, which of the following tests you meet:
I, or my professional advisor, have the capacity to protect my own interests in connection with an investment in the Company; or
I can reasonably be assumed to be able to bear the economic risk of an investment in the Company; or (c) My investment in the Company does not exceed ten percent (10%) of my net worth (or, including my spouse, our joint net worth). IN WITNESS WHEREOF, the undersigned has executed and delivered this subscription and agrees to execute such other reasonable documents as the Company may require to effectuate the intent hereof.
